The Panch Vaktra temple, also spelled Pancha Bhaktra mandir, is a 15th century Hindi temple in Mandi, Himachal Pradesh. It was one of the earliest Hindu temples built by the Sen dynasty who migrated from Bihar–Bengal region to Himachal during a period of violence and turmoil as Sultanates formed from Delhi to Bengal.

It is dedicated to Sada-shiva form of Shiva, that is the five aspects of Shiva – Sadyojata, Vamadeva, Aghora, Tatpurusha and Isana. Iconographically, it is represented with four faces at the cardinal directions and one above.

The temple has a square plan, a Nagara architecture and a notable, beautifully decorated Nandi.
