A file that you have uploaded to Wikimedia Commons, File:Lee McClure (painting).jpg, is a derivative work, containing an "image within an image". Your notification is dated 18 Dec. I emailed OTRS yesterday (20 Dec) following the "Consent" template at https://commons.wikimedia.org/wiki/Commons:Email_templates/Consent. I hope my email meets wikimedia's criteria, and that if it doesn't, it will be clear how to do so. In this case I don't see why that would be difficult: this audio file contains me playing solo electric flute. It is not now & has not ever been published by any other person or organization, nor does any other entity own the copyright.
