Pay attention to copyright

Hello, I contacted the photographer of the deleted image File:Nick DiPaolo.jpg because I cannot find the release from the photographer Eric Korenman for this image. This was a work for hire with all permissions given to Nick Di Paolo (the subject in the photo and person whose Wikipedia page the image was on). Once I receive the permission from the photographer I will forward it to permissions-commons@wikimedia.org. However, I cannot remember where to find the form I filled out in 2017 for his last image that agrees to publish it under the free license Creative Commons Attribution -Share Alike 4.0 International.
Can you send me a direct link and any further instructions I need to clear this image to go back up?

Hello, this photo was not taken from IMDB as indicated in the deletion message.  It was uploaded directly by my representative Andi Di Paolo (andreadip68@yahoo.com) with my full permission and the photographer's permission.  The image was shot as a "work for hire" by Eric Korenman for me (Nick Di Paolo - person in the photo). It is also used freely on my website (nickdip.com).  I am including an email from Eric Korenman (the photographer) releasing this photo for publication whenever and wherever I see fit including, but not limited to Wikipedia, Wikimedia, Wikicommons etc.  (see below). This photo was created specifically for my free use on the internet. I am also including the copy and pasted release from your VRTS release generator.
