Description
English: STEPS to Care is an online toolkit designed to support diverse agencies with three evidence-informed strategies to improve initial and ongoing engagement in HIV care: Patient Navigation, Care Team Coordination, and HIV Self-Management. This video demonstrates how life can be overwhelming trying to remember to take HIV medication, use condoms, pay bills, and maintain employment, but there are tools to assist and a team to help individuals conquer HIV.
