Description
English: STEPS to Care is an online toolkit designed to support diverse agencies with three evidence-informed strategies to improve initial and ongoing engagement in HIV care: Patient Navigation, Care Team Coordination, and HIV Self-Management. This video demonstrates how to navigate on the My STC Tools website to locate resources and tools available on the site.
