Question 13 needs to be rephrased.
For 14-16 How about

  • Somewhat likely
  • Likely
  • Very likely
  • Definitely
Question 17 should probably be clarified (I'm guessing the real question is something like "What did you think of the idea of a competition focused on Public Art?"
Question 19 could probably be given more options. You might be ok with info but not want a follow-up on the survey (it's also unclear whether this means getting to see the results or being asked more questions) or vice versa. I'd recommend tick-boxes for "I'm ok with being contacted about the survey" and "I would like future announcements about Wiki Loves Public Art" with username/e-mail being requested if either are ticked.

Question 10: "How hard was it for you to find lists of artworks for your city" - did you have lists only for "cities", or also towns or villages? - Or maybe better: "for the areas you were interested in?"

Questions 14 to 16: The WLM survey didn't give the intermediate values, but just the two extremes "least likely" - "very likely". I wonder whether this change was introduced on purpose. If we want to compare the values accross surveys, we shouldn't change this.

Question 13: Maybe better: "Were there any obstacles which made it harder for you or your friends to participate?" (this allows us to capture also cases of people actually not participating)

Question 19: I have elaborated on the last question - in the direction I would also like to go for the WLM 2013 survey. Please have a look and ask the chapters concerned whether they are ok with that. Make additions or remove items if required.

10: It was only cities this year, but perhaps it would be wise to use your suggestion already now so we can compare next year (if we get list of other areas than cities as well). I will change the question.
14-16: Okay, that would of course be good. So I'll change it back.
