Long Term Care refers to the traditional system of institutions and hospitals, which was upgraded to a community services systems known by hospital personal as ambulatory, non-acute community care. However, the organizations and personnel who developed the community services systems, particularly as part of deinstitutionalization (e.g., the closure of large institutions), term their community services (i.e., health and human services), "Long Term Services and Supports" (LTSS) in the Community. The Consortium of Citizens with Disabilities (CCD) confirmed the difficulty of communicating with traditional personnel regarding these changes as early as the 1990s.
