English: A waybill (internal document of the carrier) for a transport of a baby pram by train from Pardubice závodiště to Medlešice, Czech Republic. The whole document was issued in three copies: The original (prvopis) first remained with the issuing station and at month-end, it was sent to the regional accounting department (KPT). The second copy (průvodka - waybill, shown here) was held by the employee that had responsibility for the luggage (first sending station, then train crew, finally destination station). The third copy (luggage ticket - zavazadlový lístek) was first given to the passenger, who had to hand it over at the destination in exchange for the luggage. There were two main forms for luggage tickets and waybills in the 1980s. The one shown here was used by train crews or (here) in small stations.
