este sitio su superficie interna, que ningún práctico hará elec-ción de ella para perforarla. Sin embargo se presentan casos enque es menester trepanar algunos de estos sitios; v. gr. quandola situación de una fractura ó de alguna otra causa de com-presión del celebro es tal que de ningún otro modo se puedaaliviar , y que necesariamente ha de morir el enfermo si no serecurre á este medio. Entonces se deben disecar los músculos quecubren la porción del occipucio en donde se quiere aplicar eltrépano; y procediendo con mucha precaución y cuidado se pue-de hacer una perforación por medio de los senos frontales. Los instrumentos que comuntriente se emplean en esta ópe-^ración son los siguientes : la legra para quitar eí periostio, re-presentada en la Lámina XXVII, figura 3; el perforador (a)Lámina XXV, figura 3: el trephine ó abatista , Lámina XXVIfigura I í el cuchillo lenticular Lámina XXVII f figura 2: las te- (a) Trépano , taladro, modiolo. -Ej-í. xxy/i. 2,^ 2

Viniendo á la operación luego que esté colocado el enfermosobre una mesa, del modo que hemos aconsejado, y los asisten-tes tengan bien asegurada la cabeza, se descubre una porción con-siderable del cráneo al rededor de la parte que se propone tre-panar ; pero jamás se ha de seguir este método, pues aunque esnecesario quitar la suficiente porción del pericraneo para aplicarla corona del trépano con la freqiiencia que sea precisa , sin em-bargo no se debe quitar mas porción que esta, porque probable-mente resultarían largas exfoliaciones de las partes del hueso quese han descubierto, lo que retarda la curación y agrava el rigs^-go de la operación, Y así solo debemos separar con la legra la porción á^X peri-craneo que es necesaria para aplicar con facilidad el trépano; yel sitio en que ha de hacerse ha de ser el punto que
