But itspossible compression against the basi-sphenoid appears to havebeen completely neglected. My examinations (see fig. 24) haveshown that the nerve is in relation to this bone for fully halfan inch, as it passes from its superficial origin to the cavernoussinus, and that the anterior inferior cerebellar artery is dorsalto the nerve (i.e. may compress the nerve against the boneunder certain conditions) in 14 per cent, on the right side and

It is easy to see that an atheromatousvessel in this percentage might easily produce convergentstrabismus, and very deep grooving of the nerve from thatcause has been seen once in this investigation: and it is im-portant to note that in this case the artery was in the ventralposition, and had no resistant structure to compress the arteryagainst. Space does not permit further discussion of^thispoint, but it ofiers a feasible explanation of uncomplicated sixth-nerve paralysis, which has been unscientifically attributed toits long course and exposed position. Some years ago I saw a case which now appears to be probably anIllustration, as the patient suffered from chronic nephritis andarterio-sclerosis in addition to the transient paralysis of theexternal rectus of one side, which at the time of examinationdefied explanation. There was no suspicion of syphilis, and 270 Dr
