ATTENTION
  1. This passport is a State property.
  2. Except by authorized officials, any writings, prints, and/or all forms of content on this passport shall not be defaced or altered in any way.
  3. Please be reminded of Law No.12, 2006 regarding Citizenship of the Republic of Indonesia, especially of article 23 item i regarding loss of citizenship which stipulates:
    Residing outside the territory of the Republic of Indonesia for 5 (five) years consecutively not in the event of State service, without a legitimate excuse and deliberately not expressing any desires to remain being a Citizen of Indonesia before the respective 5(five)-year term ends, and every consecutive 5 (five) years from then on, the person concerned does not express a desire to remain being a Citizen of Indonesia to a Representative of the Republic of Indonesia abroad whose service area encompasses the residence of the person concerned, whereas the respective Representative of the Republic of Indonesia has notified the person concerned in writing, provided that the person concerned does not become stateless.
  4. For your interest and convenience:
    a. Please apply for Visa(s) to and seek information from the appropriate Representative(s) of Foreign Country(-ies) prior to your visit.
    b. In case this passport is lost, damaged or flawed, please report immediately to:
    - A Police Station nearby and the Immigration Office which issued this passport.
    - A local Police Station and the Head of Representative of the Republic of Indonesia nearby, in the event of loss abroad.
Description
English: Page 48 of an Indonesian passport contains official notes for use by authorities. It usually lists information about the previous passport, the bearer's occupation, and the bearer's address & contact address. The back cover of an e-passport bears an electronic biometric chip (hence marked by the biometric symbol). It also contains a notice to the bearer in Indonesian. Personal data in the official note has been edited out and superimposed by a universal mock-up identity for illustration purposes. The passport number perforated at the bottom of the page 48 has been blurred out as well.
Bahasa Indonesia: Halaman 48 dari paspor Indonesia berisi catatan resmi untuk keperluan instansi yang berwenang. Di dalamnya biasanya termuat informasi mengenai paspor sebelumnya, pekerjaan dan alamat si pemegang paspor. Sampul belakang pada e-paspor memuat chip biometrik elektronik sehingga ditandai dengan simbol biometrik. Selain itu, juga terdapat catatan penting dalam bahasa Indonesia untuk diperhatikan oleh pemegang pasport. Data-data pribadi pada catatan resmi telah dikaburkan dan diganti dengan data spesimen untuk keperluan ilustrasi. Nomor paspor yang diperforasi di bagian bawah halaman 48 juga telah dikaburkan.
