But the decomposition ofthe blood and the consequent addition of the septic element would,of course, only increase the indication for a speedy and free evacua-tion of the blood through the vagina. The present method of making a large incision and thus secur-ing better drainage in these cases of encapsulated pelvic haematoceleand haematoma and in pelvic abscesses (both intra- and extraperito-neal) pointing into the vagina, is unquestionably a great improve-ment on the old plan of puncture and small incision, with imperfectdrainage, a probable premature closing of the opening, refilling ofthe sac and the necessity for a second operation. Itis slightly tapering, and will dilate the female urethra sufficient toenable one to introduce the urethral speculum or cystoscope, oreven, if necessary, the finger. It tapers from No. 10 to No. 50French scale. The reason I had this model made was that I noticedin dilating the urethra the meatus is smaller and more resistent thanthe deeper portion. When the meatus is dilated sufficiently by aslightly tapering sound, the rest of the urethra, being elastic, willreadily allow the passage of the cystoscope. Whalebone Urethral Guide.
