L'ermita de Sant Sebastià és un edifici religiós del segle XIX a Sitges, al Garraf. L'ermita forma part del conjunt del cementiri (vell) de Sitges, per bé que té entrada independent. És de planta quadrada, d'una sola nau, amb creuer i capelles laterals. La façana és simètrica, i el temple està cobert per una teulada a dues vessants. El portal, orientat a migdia, és d'arc de mig punt adovellat en pedra. Per dessobre, la façana ostenta una obertura circular, i està coronada per una fornícula de maó i rajoles amb la imatge del sant titular. El campanar, a la part posterior de l'ermita, té coberta de pavelló amb rajoles.
