hom I am in-debted for permission to bring forward the case. For amonth he had experienced aching pain over the right brow,and a fortnight previous to his visit he discovered acci-dentally on covering the other eye that the sight in theright was very defective. The vision in this eye was 20 J.,and on examining him with the ophthalmoscope I foundthe appearances which I have represented in Plate III,fig. 1. On taking his field of vision some days later Ifound an absolute scotoma corresponding in shape andposition to the haemorrhage. Since his first visit there have been very considerable * Brit. Med. The large semicircularhsemorrhage, as well as the smaller ones on the upper halfof the circular area, underwent gradual absorption, and theretina was thrown into folds over the site of the haemor-rhage. When examined on March 31st, though there hadbeen a little fresh haemorrhage, the patients vision in theright eye was 1 J. and -;§ partly. There is not any his-tory of a blow, and the patients health has been generallygood, with the exception of what he calls ^ rheumatic gout.The first attack of this was in the large joint of the greattoe, and most of bis larger joints have been affected, butnot any smaller than the wrist and great toe. Thepatients father was gouty, and his eldest brother hasrheumatic gout. The urine is normal, the heart soundsare normal, but the radial pulse feels very firm on pres-sure.
