When the patient is seen at home, the order of examination is more frequentlyabdominal, vaginal, vagino-abdominal and, when indicated, a digital examination 14 THE PHYSICAL EXAMINATION per rectum. Inspection of the external genitals and the speculum examination areusually not required, as explained in the latter part of this chapter. However, if there are symptoms pointing to disease of the external genitals, theparts should of course be inspected. Also, in any case, if it is thought that informa-tion of value may be obtained by the speculum examination, that procedure shouldbe carried out. ABDOMINAL EXAAIINATION. Have the patient lie near the edge of the bed or table, in a comfortable position,with the head slightly raised on a pillow and the knees drawn up sufficiently to re-lax the abdominal muscles (Fig. 18).

The principal thing to determine by inspection is contour. Determine also theother items mentioned—movement of wall, color, eruption, striae, scars—butusually they are of secondary importance. As to contour, there may exist one ofseveral conditions, as follows: The smooth, moderately full contour of the normal abdomen (Figs. 19, 20,21,22).The flat, sunken abdomen of wasting disease, with emptj intestines.A swollen, prominent abdomen.
