The use of adrenalin with theordinary local anesthesia greatly pro-longs and adds to its efficiency, preventsthe pain and congestion following, andrenders the operation almost bloodless.The knowledge of the nervous anatomyof the parts is, of course, absolutelyessential, and the course of the pudicnerve and the longpudendal nerve closeto the base of the tuberosity of theischium are important. He favors theuse of Youngs tractor, and recommendsallowing sufficient time for the anestheticto act before making the incision. Withsensitive or nervous patients he finds itoften better to use a little nitrous oxidgas or primary ether anesthesia, as theinfiltrating solution can not reach theparts involved in the deeper enucleation.These parts, however, are supplied bythe hypogastric plexus of the sympathicand the discomfort is not necessarilygreat. He reports a case in which hethinks this method of operation wasdirectly life saving.
