104 Behavior of the Stomach in Ulcer and Cancer of the Duodenum marked distention of the duodenal bulb, suchas to overlap and conceal the pylorus. In theplate made of the abdomen preceding theserial plate and also preceding the administra-tion of the thin barium suspension (the thirdbarium meal) the pylorus appears widelyopen and the absence of an organic obstruc-tion at the pylorus is thus demonstrated. Innone of the plates is there a good delineationof the duodenum beyond the duodenal bulb.A study of the outline of the stomach does of the barium from the stomach rather thanto any affection of the small intestines. Theabdomen was palpated beneath the .v-rayscreen. No area of pain on pressure and noabnormality of the colon were thus detected. No gallstones can be detected on any of theplates. This, of course, does not exclude them,because in the majority of cases gallstones aretransparent to the .v-ray. Mr. T. was recalled on the 16th and acomplete set of tooth films made. These films

Patient prone.Two days alter the second barium (suspension) westill find a large gastric residue, an open pylorus, afilled bulb and a delineation of the colon. Pyloricobstruction is excluded by the last two plates. not show any filling defect or contractureindicative of gastric ulcer or gastric cancer. Onone of the serial plates there is a deformity inthe usual outline of the pylorus, but this wasnot repeated in other plates. On all of the plates there is seen to be adiminution in the normal size of the liver asestimated by its proportion to the rest of thebody. The spleen is somewhat enlarged. Thereis no enlargement of the head of the pancreas,judging from the position of the duodenal bulband from a lew fragments of the arc ol theduodenum. The retention of barium scattered throughthe small intestines thirty-six and forty-eighthours after the administration of the bariummixture we would attribute to the slow escap
