PREVIOUS to last year, it had been ourcustom to interpret all annular shad-ows on chest plates as cavities. In reportingseveral thousand cases during the preced-ing years, we had never questioned that anarea of decreased density surrounded byan area of increased density should beconsidered other than a cavity. Of course,we had recognized pneumothoraces of thelarger type, which were at times clinically which we described as atypical of a cavity, but no suggestion was made that it mightbe a localized pneumothorax. There was asmall amount of fluid present in the cir-cumscribed area, this supporting the viewthat the condition was one of cavity forma-tion. Plates made several weeks later ofthe same case showed a marked diminutionin the size of the supposed cavity andshowed the absence of fluid. This of neces-

We believe that the sameinterpretation of these shadows was madeby other laboratories, because a completereview of the subject and a study of a largenumber of reproductions of chest platesdid not reveal where any reference wasmade to a localized pneumothorax. A review of roentgenograms recentlypublished by Dunham in the Stereo-clinicfailed to show that this question had beendiscussed, and in fact several cases de-scribed as showing cavity formation werein reality cases of localized air pockets. In making a report on a chest examina-tion, last fall, we noted an annular shadow *Read at the Nineteenth Annual Meeting of The Americ.vn Fig. 2.
