the drainage tube may be inserted. Even yet,in certain text-books, the student is directed to make the smallest possibleskin incision in cutting down to find an artery. Such a procedure is no longerlegitimate. The true surgeon makes a free skin incision, but proceeds, as carefullyand sparingly as he is able, to make the deeper dissection. A large skin incision pro-duces no additional injury worth mentioning as compared with a small one, because,when carefully sutured, it heals with equal rapidity and certainty ; and further, thelarger size of the resulting scar is of no significance, provided it runs in the properdirection. This brings us to that consideration which we have for years regarded asessential in the arrangement of incisions. According to the investigations of Langer upon the planes of cleavage of theskin, the tension of the skin is very different in different directions, and two incisionsat right angles to one another display a very different amount of retraction of the 42
