as a basis,we may gradually build up the larger groups necessary for moreextensive operations. 2. General Abdominal Set.—This outfit may be consideredas a dissecting set, sufficiently augmented (Fig. 121) to permit ofan exploration of the abdominal cavity. The number of arteryclamps is increased to twelve. To these are added six medium-sized, curved clamps. In addition to the smaller-sized dissectingforceps, one long thumb forceps is included for the proper placingof pads for packing off the intestines and for such other uses asmay require intraperitoneal manipulation. The retractors areincreased by the addition of sets of two larger sizes than thoseused in ordinary dissections. The needles must include somefine, round ones for visceral repair and the suture material shouldinclude (for the same purpose) fine catgut, fine silk, or fine linenthread—possibly all three. This may be considered a set thatwill suffice for an exploratory laparotomy, but that must be 323 324 THE OPERATION

With this as the basis for abdominal work, we can proceedto the sets formed around it for operations upon special regionsand conditions. 3. Appendix Set.—The instruments necessary for operationare identical with those of the general abdominal set, with oneor two possible additions. There may be (in addition to the twodissecting forceps already mentioned) one smooth thumb forcepsfor use in inverting the stump of the appendix. It is possiblethat a special clamp may be used for crushing the appendixbefore amputation and an actual cautery (either Paquelin orelectric) for cauterizing the stump. 4. Gallbladder Set (Fig. 122).—The gall-bladder set, also,is identical with the general abdominal outfit, certain additionsbeing necessary for operations upon this organ.
