Foi construído com os donativos da população de S. João da Madeira, fruto da vontade de uma comissão constituída para o efeito em 1930, tendo sido inaugurado em 6 de Novembro de 1938, em ato presidido pelo Bispo do Porto. Situa-se num espaço verde denominado por Parque Nossa Senhora dos Milagres, um dos mais apreciados parques urbanos da cidade. Reporta-se ao período neorromânico, tendo na sua fachada uma imagem de Cristo crucificado, em mármore.
