English: Sagamore of the Wabash award bestowed by Governor Joseph E. Kernan to Daniel Mark Cundiff on January 9th, 2005. The paper is off-white, softly textured, and lightly corregated. It appears to be printed or silk-screened (except for the ribbon which is an embossed reflective gold material. There is nothing on the back side of the certificate. The signature is an actual signature from the Governor. According this version of the certificate, at the bottom it says "STATE FORM 5775 (R4/9-03)". |

Scanned in 300 DPI on a Canon CanoScan 5600F. The image is the spliced results of three separate scans due to the size of the document. The editing was done in Gimp 2.6.11.
