No. 293) courtesy of the Provincial Archives in Opawa with note: "According to the remark in the Geburtsmatrikel the change of name under the number 26 681 of the Silesian provincial government was made in 1906. The surviving records of the name changes are stored in the current inventory of the Silesian State Government / Zemskå Vlada Slezska under the signature No. 42, inventory number 1150. The files are organized by individual years and boxes 588, 589 and 590 are where the files from the years 1905, 1906 and 1907 are preserved. The documents for name change Kusionowicz - Grodynski are not among them. In Einreichsprotokoll the Silesian provincial government in 1906 (PLN 24001-30000), it has been verified that the Namensänderungakt No. 26 681 on 07.09.1906 filed with the Silesian Provincial Government on 08.09.1906 and as already done at the district office was passed in Bielsko. Among the acts of that authority, the act must therefore be sought. The Bielsko district then in Austrian Silesia is now part of the Polish town of Bielsko-Biala."
