Polyurethane crystals, mostly triangular, in pericapsular breast tissue adjacent to a polyurethane-coated breast implant with a prominent foreign body giant cell reaction. The implant was removed because of infection. The gross photo is not from this case. Silicone breast implants may have a smooth or textured surface or be coated with a polyurethane shell. The latter has been found to be associated with a significantly lower risk of capsular contracture and a superior aesthetic outcome compared to other types of implants. Studies, however, have shown potential toxicity with polyurethane and that it tends to disintegrate. Polyurethane implants have not been reintroduced in the US since their discontinuation in the early 1990’s.
