Text Appearing Before Image:
pex there is heard a systolicand diastolic murmur, the former transmitted into the axilla; systolicand diastohc murmurs also heard over the third left interspace andtransmitted down the border of the sternum. Pulse is regular, soft,full; not typical Corrigan in character. Lungs: Normal. Liver:Normal. Spleen not made out. Extremities: There is pain and ten-derness over dorsum of left foot, also in joints of left hand. There isalso pain over left arm as high as the shoulder. August 4. Patient had a sharp chill this a.m., followed by rise oftemperature to 104°. Spleen not made out. Urine is normal. Blood :No malarial organisms found. Leucocytes, 10,000 per c.cm. August 14. Patient has been running a very irregular temperature,ranging from 98.8° to 104°; she perspires profusely. Heart murmursare heard more distinctly; there are twenty petechias. Pulse is moreCorrigan in character. Blood-culture: No growth in two differenttrials. No malarial organisms found. Leucocytes, 10,000 per c.cm.

A REPORT UPON MALIGNANT ENDOCARDITIS. 295 August 30. Patient has had a chill every day, with a rise of tem-perature to 106°, accompanied by profuse sweating. She is dehriouspart of the time; no petechiae have appeared. September 15. Chills continue every day, temperature rangingfrom 99° to 106° F. almost daily. Heart: There is a distinct systolicmurmur heard over the entire precordium. Anaemia is profound; nopetechial spots present; patient is in extremis. Urine contains a traceof albumin and a few hyaline casts. Blood : Hemoglobin, forty-fiveper cent. Leucocytes 16,000 per c.cm. Blood-culture: No growth onfour different trials. Autopsy.—Heart: Weight, eleven and one-half ounces. Pericardiumcontains a little turbid serum. Valves: Aortic has one cusp greatlyhypertrophied like a polypoid mass of granulation tissue; surface a littlerough, but no ulcerations. Muscle is pale and looks fatty. Lungs :Congested. Liver: Fatty. Spleen shows a few small infarcts. Kidneyshows moderate inters
