f the enlargement is obscure; in certaincases it seems due to excessive sexual excitement, but it is hardlyever met with in connection with urethritis. The anatomical changes present are of two kinds, either a diffuseform of cell proliferation in the inter tubular connective tissue,commencing at the surface and dipping in towards the centre ofthe organ, or the formation in the body of the testis of gummynodules, which subsequently become fused together. In the earlystages one sees the delicate connective tissue to be crowded withspherical corpuscles, resembling leucocytes, separated by a scantyamount of homogeneous material; but as the disease progressesthe corpuscles fattily degenerate, whilst the intervening substanceincreases in density, and presents the appearance of a tough 26 ORCHITIS. fibroid network. These degenerative changes commence in thecentral portions of the nodules, the resulting tough yellow massesbeing surrounded by a vascular zone of actively proliferating Fig. 173.

As the affection subsides, this vascular zone, by anincrease of its fibroid over its corpuscular elements, forms a toughcapsule to the now cheesy encysted mass. Meanwhile, however, the changes in the connective tissue, bypressing upon the tubules of the testicle, cause a thickening oftheir coats and a fatty degeneration of their lining epithelium,so that in advanced cases of the disease the secreting power isgravely impaired, and the physiological activity of the organ diminished, or wholly destroyed. In-IlG. 174. deed in some instances, after all active inflammatory changes have subsided,the testicle gradually dwindles away toa hard fibroid mass, not larger than ahazel nut, and of course destitute offunctional power. This form of diseasedtesticle is apt to be associated withhydrocele. Scrofulous orcliUis, although herealso similar changes in the connectivetissue and the tunics are met with,presents some striking contrasts
