Мемориальный знак устанавливается на фасаде дома, адрес которого стал для этого человека последним. English: The Last Address Sign is a commemorative plaque that is installed on the last known residential address of victims of the Soviet Union's Communist regime. The memorial sign is a small, 11x19 cm stainless steel plaque that contains information about the repressed person. This information usually contains his or her name, profession, date of birth, date of arrest, date and reason of death (was shot or died in a Soviet concentration camp), and date of rehabilitation ('for lack of corpus delicti' ).

Москва, Мясницкая улица, дом 15 (в 1935—1990 годах — улица Кирова).
В этом доме в годы советской власти жили и были расстреляны палачами НКВД:

• режиссёр театра «Скатувэ» В. Х. Форстман,
  
• бухгалтер Н. А. Галкин,
  
• служащий П. М. Веселовский,
  
• полиграфист М. К. Подземский,
  
• экономист Е. Т. Властовский и
  
• священник В. С. Елпатьевский.
  

Все они были посмертно реабилитированы «за отсутствием состава преступления».
В память об этих людях в воскресенье, 6 декабря 2015 года, на этом доме были установлены мемориальные знаки «Последний адрес».
