e fundus the growth has invaded the uterine wall ; the lower part projects into the uterine cavity. and subserous, and under these circumstances they may attain con-siderable dimensions, the largest on record weighing forty pounds(Piquand). A point of clinical importance is the liability of thesegrowths to become polypoid. Fig. 255 shows such a one from whichthree polypi were removed prior to hysterectomy. In a number ofthese polypoid cases inversion of the uterus has occurred, inversionbeing more common in association with sarcomatous than with myo-matous polypi. It has been suggested (A. R. Simpson) that inversionis favoured by malignant infiltration producing a softening andparalyzing effect on the uterine muscle. Circumscribed sarcoma of thecervical wall is rare. Figs. 257 and 258 show the naked-eye and micro-scopic features of such a growth. (c) Sarcomatous Metaplasia in a Fibromyoma.
