Polski: Kopia dokumentu znalezionego w kopule wieży kościoła bł. Michała Kozala w Gnieźnie. Dokument z 1894, odnaleziony i oddany do Muzeum Archidiecezjalnego w Gnieźnie 1 grudnia 2000 roku.

Tłumaczenie: Kaplica Prowincjonalnego Szpiala dla Chorych Psychicznie "Dziekanka-Gniezno" Wybudowana w roku 1894. (...) zarządu Prowincji Poznańskiej. Naczelny prezes Prowincji Poznańskiej: Dr von Dziembowski, radca prowincjonalny Wolff. Dyrektor szpitala: dr Kayser. Szpital dla obłąkanych został zbudowany zgodnie z planem dyrektora dr Kaysera, zaprojektowany i zbudowany przez Inspektora Budowlanego Brauna, projekt kaplicy wykonał technik budowlany Bruno, zgodnie z wytycznymi Inspektora Budowlanego. Prace murarskie wykonali: mistrz murarski Hoffman z Gniezna i majster Kinzell, drugim majstrem był Zelm. Roboty ciesielskie wykonywali przedsiębiorca budowlany Koch(...)smann z Rosenbergu i podmajster Timm. (...) Dokument został dzisiaj umieszczony w kopule wieży. Dziekanka - Gniezno
