ight plunger fits accurately in thisinner tube. An ointment may be placed in this tube, and thewhole encased in the outer one. This instrument is so arrangedthat when it is carried down to any desired spot, by turning theinner tube and forcing a little gauge pin into a slot in the outertube, the fenestrum is brought opposite the perforations in theouter tube. By gently forcing the piston rod down, an ointmentmay thus be applied in any quantity to any particular spot orspots within the urethral tract. The instrument may be usedequally as well for a syringe. It is the best one I have ever usedfor medicating the urethra. The surgeon can with positive cer-tainty know exactly where he is applying his remedies, and hasperfect control of such agents, medicating only such spots as hemay choose, applying large or small quantities as the case maydemand. Any surgeon who is familiar with the anatomy of theparts, knows that a perfectly straight instrument can be readilypassed into the bladder. A A

Its chief use is in cases of acute gonorrhoea, but it also finds appli-cation in the chronic state, with accompanying erosions, rents, broad superficialloss of substance and ulceration or croupous mfiltration of the mucous membrane. The customary method of using injections, suppositories and ointments, isundoubtedly deficient in not providing for a preliminary thorough washing ofthe affected tract, and the want of care and exactness, which lies in applyingmedicaments over an unremoved layer of inflammatory products, needs hardlyto be mentioned as incompatible with our ideas of antiseptic treatment.
