Français : Diagnostic original : Thrombus pariétal et thrombus en grelot sur anévrisme du ventricule gauche consécutif à un infarctus. Diagnostic actualisé : Anévrisme ventriculaire gauche thrombosé. Histoire : Homme de 51 ans admis à l'hôpital pour insuffisance cardiaque, dyspnée d'effort et de décubitus. Infarctus myocardique 6 mois auparavant. Description macroscopie : Cœur de 500 gm. Il y a symphyse péricardique à la face antérieure du ventricule gauche. La face externe du ventricule est violacée avec des petits points rouges. À la coupe, le myocarde est pâle et mesure 8 cm d'épaissseur à cet endroit. Du côté de la lumière, il y a un thrombus jaune pâle lamelleux de 2.7 cm avec un thrombus rouge noir plus récent de 4 X 4 cm. Description microscopie : Un prélèvement fait au ventricule gauche présente une sclérose intense avec quelques amas lymphocytaires, délimitant des petites plages de tissu musculaire. Sur un endroit, il y a du tissu musculaire en voie de nécrobiose. L'endocarde est recouvert d'un large thrombus en voie d'organisation. Année du prélèvement : 1954.
