Theprinciples borne in mind are to remove the sac at thehighest possible level with the minimum disturbance oftissue. The cutting and interference with tissues hagbeen reduced to such a degree that verj rapid convales-cence follows, and tbe reaction associated with herniaoperation is obviated. The advantages of an operationwhich reduces the cutting of tissues down to an inchincision in the skin and superficial fascia, and a smallincision in the spermatic fascia, will be apparent. Nodissection of tissues is undertaken; just as Matas in hisoperation for aneurysm avoided injury and disturbance ofsurrounding structures by attacking an aneurjsni fromwithin the sac, we avoid dissecting, cutting, or injuring thedelicate or important structures which surround a hernialsac by attacking the hernia from within the sac.
