Description

In the absence of estrogen stimulation, squamous epithelium of the exocervix and vagina fails to mature, leaving a thin layer of basaloid cells with high nuclear/cytoplasmic ratio. On Pap, these can look very much like (or identical to) cells of high-grade dysplasia. For this reason, I rarely make a straight-out call of HSIL on an initial Pap from a postmenopausal or postpartum woman. Rather, I call it ASC-US and request a course of local estrogen cream prior to the next Pap. After this treatment, atrophic atypia will resolve completely, while true dysplasia won't be phased.
