d. Desmarres retractors,as shown on Plate 9, are specially recommended to begin-ners, because an inexperienced operator is in great dangerof cutting the lids in this operation ; but specula like thoseshown in Figs. 14 and 33 can also be used. Before beginning, especially if a general anesthetic hasbeen given, the operator must carefully notice once morewhich eye is to be enucleated, to guard against the accidentof removing the wrong one, which has happened morethan once. The mistake is specially apt to happen whenthe eyes present the same external appearance, as whenboth are normal so far as external appearances go, orequally inflamed, as, for example, in sympathetic disease.Sometimes the mistake is due to the fact that the operatortakes his position at the patient \s head. Beginners ought tomake it a rule to have the sound eye covered for some timebefore the operation with an occlusive bandage. (A markwith an anilin pencil should be made on the brow, overthe eye to be removed.—Ed.)

268 OPERATIONS ON THE EYE. Plate 9. Enucleation of the eyeball. The globe has been rotated to the extremeright aud the optic nerve is being divided with the scissors. It is hardly necessary to say that the parts around theeye and the conjunctival sac must first be cleansed andthe instruments boiled. It is not necessary to cut off theeyelashes or shave the eyebrow. If the operation is performed under local anesthesia, astrabismus-hook for finding the muscle tendons should beused, the operator proceeding as follows : In enucleationof the right eye he takes up his position on the right sideof the patient, who lies on the operating table; in case ofa left enucleation he stands at the patients head.
