L. 118 CLINICAL MICROSCOPY. SPEEMATORRHCEA (Continued). The form of spermatorrlicea wliicli is represented on Plate Q^,the last drops of urine passed during the stool having a milkycolor and glutinous consistence, is at times diagnosed by the pa-tient himseK; he usually asserts that he observed this peculiarliquid accidentally. On taking this drop between the lingers, itgives a fatty, soapy sensation; otherwise it resembles the semen.These drops, which, under the microscope, represent pure seminalfluid, pass occasionally, but rarely, at the heginning of the stool,when the abdominal pressure commences; but generally they ap-pear at the close, when the final contractions are made by thesphincter ani, and thereby the seminal vesicles compressed. Inrare instances this seminal fluid passes some time after the comple-tion of the stool, and when the patient is already dressing. Thisoccurs, then, with more or less disagreeable, even painful sensation;in one case I observed even a chill. PLATE

In a large number of casesof spermatorrhoea which I have examined I have observed thisoccurrence four times. Plate 64 represents a specimen from thecase of an unmarried young merchant, who consulted me on ac-count of chronic diarrhoea and loss of flesh. Chemically, I foundnothing abnormal in the urine, but with the microscope I dis-covered a moderate spermatorrhoea with hyaline cylinders, a littlebroader than the renal cylinders. Not knowing of the existence of hyaline cylinders in sperma-torrhoea, I thought I had to discover a disease of the kidneys, andmade the most careful search in that direction. In the case of a tailor, thirty-eight years of age, with spinalirritation and total impotence, these cylinders
