e was aconstant dropping of urine from the vagina, and forsome time 1 was puzzled to ascertain whence itcame. At last I passed a bougie into the os uteri,and upon its withdrawal the urine followed quite free-ly. This, at once solved the mystery. The water es-caped from the bladder into the cavity of the womb.Eighteen months previous to this date, she had beendelivered with instruments by a bungler, which hadcaused this sad result. I considered the case hope-less, so far as the resources of our profession wereconcerned—it being quite beyond the reach of anysurgical operation. I told her that if she ever recov-ered, it would be by an operation of nature alone. Iplugged up the os uteri for an experiment, and solong as she could bear the tent to remain, the urinepassed through the urethra. So much inflammation,however, was produced, that it could only be toleratedfor a few days. The last time I saw her she said thatnature was at work for her, and that she was gettingsomewhat better. qH=

SEMINAL DISEASES. 225 USE OF BOUGIES. Whenever bougies are indicated in the treat-ment of seminal diseases, unless they are com-plicated with stricture, it is very seldom that asmaller size than number seven will be re-quired. The instrument should be large enoughto fill and produce gentle pressure upon theprostatic portion of the urethra. The fusiform or bulbous bougie is betteradapted to both these cases, and many kinds ofstrictures, because we obtain pressure justwhere we need it, without fatiguing the otherportions of the urethra. In cases where there is much tenderness orpain caused by their introduction, number sevenshould be used first, and the higher numbersgradually succeed each other. The instrumentsin Plate VII. are reduced to about one half theirproper size. CAUTERIZING INSTRUMENT. T;>on Plate VII. may be seen the only properinstrument as yet known for cauterizing themouths of. the ejaculatory ducts. Lallemandsliorte-cau.stiqiie for this purpose is liable to 226 BOSTWIC
