The Thomas knock-knee brace. Thomas knock-knee braces with pelvic band. Thepelvic band may be divided also, the two parts beingjoined by straps (Fig. 344). the limb being fully extended so that the deformity is made asextreme as possible. One hand then clasps the knee, the pal inlying against its inner aspect ; with the other the calf is graspedfirmly and the leg is then gently straightened over the fulcrumformed by the palm of the hand, and is held in the correctedposition for a moment. This manipulation should be continued DEFORMITIES OF BOXES OF LOWER EXTREMITY. 569 with gradually increasing force, although not to the extent of caus-ing actual pain, for ten minutes at least twice in the day andoftener if possible. This serves as an indication for making the soles ofthe shoes thicker on the inner border, as in the treat incut of flat-foot, in order to throw the strain upon the outer border of thefoot. The patient should be instructed to walk with the feet parallel with one another, and for older children the tip-toe exer-cises, in which the body is raised upon the toes as many timesas the strength permits, or games or exercises in which the legsarc extended should be encouraged. Such exercises are oftenefficacious in the early stage of adolescent knock-knee, for, ashas been mentioned, genu valgum is an exaggeration of the atti-tude of rest; therefore, its progress should be checked by theassumption of the attitudes proper to activity. Bicycle, andparticularly horseback, riding may be recommended also in thisclass of cases.
