Description	A U.S. Navy Douglas A-4E Skyhawk (BuNo 152091) from Attack Squadron 106 (VA-106) "Gladiators" is positioned for launch aboard the aircraft carrier USS Intrepid (CVS-11), in September 1968. VA-106 was assigned to Attack Carrier Air Wing 10 (CVW-10) aboard the Intrepid for a deployment to Vietnam from 4 June 1968 to 8 February 1969. On 23 September 1968, the A-4E 152091 suffered a generator faliure and was forced to return to the carrier. Shortly before landing, the aircraft banked left, dropped its nose, hit the LSO-platform and crashed into the sea. The pilot, LCdr. Francis Callaghan, was killed.
