中文（臺灣）：艋舺清水巖俗稱祖師廟，為第三級古蹟，位於萬華區康定路81號，始建於清乾隆五十二年〈西元一七八七年〉，奉祀清水祖師陳昭應，為福建安溪移民，自原籍湖內鄉清水本巖分靈而來。嘉慶二十二年〈西元一八一七年〉重修，咸豐三年〈西元一八五三年〉「頂下郊拼」時毀於戰火，至同治六年〈西元一八六七年〉又重建。其後屢經翻修，迄今大體能保持重建原貌。
